Florida Eye Custom Vision, formerly Maida Custom Vision, has a rich history of providing world-class vision correction in Northeast Florida. The practice was originally formed in 1995. In 2016, the center merged with Florida Eye Specialists, bringing on esteemed surgeons Dr. Amit Chokshi and Dr. S. Akbar Hasan. Today, our St. Johns office serves as a premier destination for advanced vision correction.

Our practice was built upon the pioneering work of Dr. Jerry Maida, who formed his refractive practice in 1995 and solely dedicated it to laser vision correction. Dr. Maida was recognized worldwide as an authority on refractive surgery. He was the first to perform the LASIK procedure in Northeast Florida in 1996 and utilized the Excimer laser for over 25 years.
Throughout his distinguished career before retiring from the operating room, Dr. Maida performed more than 22,000 LASIK procedures and 37,000 eye surgeries overall. His dedication to excellence earned him the rare National and International Summit/Autonomous Excellence awards. While Dr. Maida no longer performs surgery, he remains actively involved in the practice for patient evaluations and post-operative care, ensuring the standard of excellence and patient-first care he established remains the cornerstone of Florida Eye Custom Vision today.
